1. Items necessary for hospitalization
Please remember to label your belongings and, as a general guideline, keep them within your possession. We cannot be held liable for any damage or loss.
- Please bring the following daily necessities with you. You can also purchase these items at the hospital's convenience store. Items available for purchase are marked with a basket.
- If you are scheduled for surgery, please follow the instructions provided by the medical department.
- We kindly request that you limit the amount of luggage you bring, as storage space is limited.
- Bedding will be provided for your stay at the hospital.
- To make your visit more convenient, you can use the "hospitalization set," which includes "towels" "nightwear" and "disposable diapers" allowing you to come to the hospital without bringing extra items.
- Disposable diapers are available at the hospital for a fee.
For safety reasons, please use items provided by the hospital whenever possible. - Chopsticks and spoons will be provided by the hospital for your convenience.
- Bringing dangerous items into the hospital is strictly prohibited.

We recommend wearing shoes with rubber soles and comfortable sneakers in the hospital.
Please avoid wearing slippers, Crocs, flip-flops, or similar footwear, as they may cause tripping hazards.

2. Hospitalization rules
The following rules must be obeyed during hospitalization。
Smoking, including e-cigarettes, is prohibited in the hospital
Alcoholic beverages, including non-alcoholic beverages, are prohibited on hospital grounds.
Call and photographing are prohibited outside of the hospital's usable areas.

- Please refrain from doing anything that may disturb other patients.
- Going out or staying out overnight is prohibited in principle.
- Please watch the TV or listen to the radio with headphones (not necessary in private rooms, but please refrain from making noise that disturbs others). Please refrain from using the TV or radio after the lights are turned off.
- If you'd like to bring in any electrical appliances, please reach out to the Admission and Discharge Administration Center. (Please apply for this service separately)
- Please keep your belongings safe to prevent theft. Please keep the amount of cash and valuables you bring to a minimum, and always put them in the bedside safe and lock it. In the event of theft or loss, the hospital will not be held responsible.
- Please do not attach any notices or hooks to the fixtures or walls of the hospital room. If any property in the hospital is damaged, you may be required to pay for the repairs.
- Depending on your condition, you may be asked to change rooms, whether you are in a private or shared room.
- About the opening and shutting of the private room window of the sickroom of our hospital, opening and shutting within 110mm is enabled in the structure of facilities. Please be careful enough so that the fall of the child, the fall of the thing do not occur.
- Food is not allowed to be brought into the hospital or exchanged among patients for the prevention of food poisoning and dietetic treatment.
- We strictly refuse any gratuities to our staff.

3. About meals
- Meals are planned and provided by a registered dietitian based on your doctor's instructions.
- Please do not order or bring in outside food.
- If you have a food allergy, consult your doctor or nurse for guidance.
- To avoid food poisoning, eat your meals as soon as possible.

4. About Visitation

At our hospital, visiting procedures require a reservation. Please make a reservation through our online portal.
※Priority will be given to those who have paid the private room fee.Visiting restrictions are in place as a measure against COVID-19 infection.

Visiting hours and visitors vary for the ICU, SCU, Pediatrics, and Obstetrics departments, so please check the website for details and speak to staff in each ward.
If you need to visit outside of opening hours in an emergency, please speak to the Emergency After-Hours Reception and Security Guard Room on the 1st floor of the South Wing.
- Please note that we may refuse visitors depending on the patient's condition.

Bringing food and drinks into patient rooms is prohibited.
Alcohol, tobacco, and other dangerous items are strictly prohibited from being brought into the hospital. If discovered, they may be disposed of by the hospital.
- Please refrain from visiting if you are feeling unwell or under the influence of alcohol.
- Only three visitors (junior high school students and older) are allowed at one time, and visitors are requested to keep the visit short, approximately 15 minutes.
- Depending on the infectious disease outbreak, visiting may be banned.

5. About visiting and making phone calls (Privacy Protection)
Our basic policy regarding personal information is posted on the digital signboard. From the perspective of protecting patients' personal information, if you have any questions or concerns regarding visitation or phone calls, please contact the SOUTH WING 3F Admission/Discharge Center or reception.

6. About the people accompanying the patients
Chaperones are not permitted except in special cases. However, only family members may accompany the patient if they have permission from the attending physician due to the patient's medical condition. In this case, please fill out the "Request for Permission to Accompany form" to obtain permission.

7. Hospital facilities
Bedside Table: Tablet(TV), Refrigerator, Internet
- Bedside table can be rented for 660yen/day.
- Tablet (TV)/refrigerator usage fee X number of days of usage. This will be billed together with the hospitalization fee.
- The patient can apply for the bedside table at the Admission and Discharge Center on the 3rd floor of SOUTH WING.

9. Restricted areas
Do not enter or leave other patient rooms, ward staff stations, or other administrative areas without permission.

10. Emergency response
- Please check emergency exits and emergency stairways upon admission.
- In the event of an earthquake, fire, or other emergency, please follow the guidance of doctors, nurses, and other hospital staff. Please refrain from using the elevator for evacuation.

11. Nuisance, Verbal abuse, Violence, Misconduct, or Harassment of staff
- Please refrain from doing anything that may disturb other patients and cooperate with us in order to make your stay in the hospital a pleasant one.
- Verbal abuse, violence, misconduct, and harassment are prohibited in the hospital. Excessive violence and verbal abuse will also be reported to the police.

12. Visits to other hospitals during hospitalization (excluding dentistry)
- During hospitalization at this hospital (including outpatient and overnight stay), patients are not allowed to visit other hospitals. If you need to see another hospital, please consult with your doctor or nurse.
- Please do not receive medications from your family hospital or other hospitals while you are hospitalized, as you will not be able to claim insurance for medical expenses at other hospitals. It is not possible for a family member to pick it up on behalf of the patient.

13. For better medical care
Informed Consent (Explanation and Consent), Second Opinion, and Medical Record Disclosure
We try to provide clear and detailed explanations of treatment in writing as much as possible. We then listen carefully to the patient's opinion and together we decide on a treatment plan. We also guarantee a second opinion system and disclosure of medical records.
Use of patient critical path (treatment plan schedule)
Patients may be given a treatment plan schedule (critical path) prior to the start of treatment for the purpose of sharing information about the treatment between the patient and the medical staff. We also develop nursing plans that incorporate the needs of patients and their families.
